How
to Register
All participants are kindly requested to register in advance
by completing and submitting the Registration Form and
by paying the appropriate fees. Please note that the Registration
Form is to be used to register one participant only and,
if applicable, his/her accompanying person(s).
Individual registration of the accompanying person is
not allowed. Registration via the Online Registration
Form is highly recommended. For those not wishing to register
this way, registration also will be accepted by fax or
airmail to the Registration office and become valid upon
confirmation of the registration fee transfer received
by the Registration office.
Online Registration
Please use the
Online Registration Form
to register. Each participant registering online will
receive a
PRN (Personal Registration
Number) at the end of registration procedure.
Please make a printout of your Registration Review where
you will find your PRN! An email with the registration
details will be sent to each participant not later than
one day after submitting the Online Registration Form.
If you do not receive the email please contact the Registration
office.

Form in word format is to be used for registration by
fax or e-mail only. Each participant registering will
receive an email with the registration details and PRN
(Personal Registration Number) to be included in details
of payment not later than one week upon submission of
the Registration Form. If you do not receive the email
please contact the Registration office. Please
do
not send any payments without including the PRN
in the details of payment. Such payments cannot be identified.

A confirmed banker's cheque in US dollars should
be sent to the 5th ICSC Registration office by mail,
together with the PRINT or COPY of registration
form |

* All bankremittance charges are to be paid by registrants.
- Sender's name should be the participant's name.
If the sender's name is different from the participan's,
; you should send the wire transfer slip to registration secretariat by fax.
- A COPY of the REMITTANCE RECEIPT and Registration Number should be sent through fax or Email to the Registration Secretariat.
- If registrants are willing to take a student discount, they are supposed to send student ID or alternative documents to the Registration Secretariat by E-mail or Fax.

Cancellation Policies
- All cancellations
must be submitted in writing to the Registration
Office.
- Please indicate your
name, registration name, first name and registration
number |
| Cancellation made before 13 Feb 2008: |
refund 25% processing fee |
| Cancellation made after 13 Feb 2008 |
No registration refund will be issued after this
date. |
| As an alternative to cancellation,
your registration may be transferred to another
person prior to 13 Feb 2008 without penalty - on
or after 13 Feb 2008, a US$ 50 penalty will be incurred |
